Registered Nurse | Case Manager 

Role Description

This is a full-time on-site role for a Registered Nurse at BRIDGE Hospice in Lubbock, TX. The Registered Nurse will be responsible for providing direct patient care, assessing patient conditions, administering medication, monitoring vital signs, and collaborating with a multidisciplinary team to develop and implement patient care plans. The Registered Nurse will also provide emotional support to patients and their families, educate patients on their condition and care, and ensure the delivery of high-quality and compassionate hospice care.


Qualifications

  • Valid Registered Nurse (RN) license
  • Experience in hospice or palliative care
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and interpersonal skills
  • Ability to work effectively in a multidisciplinary team
  • Empathy and compassion for patients and their families
  • Knowledge of end-of-life care and symptom management
  • Ability to provide emotional support and counseling
  • Bachelor's degree in Nursing (BSN) is preferred
  • Certification in Hospice and Palliative Care Nursing (CHPN) is a plus
