Starting October 1, 2025, all Medicare-certified hospices, including Bridge Hospice, will be required to use the new HOPE (Hospice Outcomes and Patient Evaluation) Assessment. This tool replaces the current Hospice Item Set and aims to improve patient care by collecting standardized data on symptoms and outcomes at multiple points during the first 30 days of hospice care. By gathering more timely and detailed information, the HOPE Assessment helps hospices respond more quickly and effectively to patient needs.
