Certified Nursing Assistant
We are seeking a Full-Time CNA to provide compassionate personal care and support to hospice patients in the home setting. Under the supervision of an RN or therapist, this position assists with personal care, mobility and transfers, ambulation, range-of-motion activities, and other services that promote patient comfort, dignity, and a safe environment. The CNA/Home Care Aide also observes and reports changes in patient condition and works closely with patients, families, and the hospice care team.
Qualifications
- Current certification/credential required to practice as a CNA/Home Care Aide
- Compassionate, dependable, and comfortable providing hands-on personal care
- Ability to follow the patient’s established care plan and RN direction
- Strong communication skills and ability to recognize and report changes in patient condition
- Ability to safely assist with mobility, transfers, ambulation, and personal care
- Ability to meet the physical demands of the role, including frequent standing and walking and lifting/carrying associated with direct patient care
- Ability to travel to patient homes and care facilities
Assistant Director of Nursing (ADON)
We are seeking a Full-Time Assistant Director of Nursing (ADON) in hospice care to support clinical leadership and ensure high-quality, compassionate care for patients with life-limiting illnesses in their homes or care facilities. This Registered Nurse (RN) leadership position assists with overseeing hospice nursing services, supervising clinical staff, coordinating individualized patient care plans, and ensuring compliance with state and federal healthcare regulations. The Assistant Director of Nursing will collaborate with the Director of Nursing and interdisciplinary hospice team, review clinical documentation, support staff training and scheduling, monitor quality of care, and help ensure patients and families receive coordinated, dignified, and compassionate end-of-life care among other duties.
Qualifications
- Current, active Registered Nurse (RN) license in the state of Texas
- Certified in Basic Life Support (BLS)
- Preferred previous nursing experience, especially in hospice or palliative care
- Prior leadership, supervisory, or nursing management experience preferred
- Knowledge of hospice care standards, clinical documentation, and applicable regulatory requirements
- Strong communication, organizational, and clinical assessment skills
- Ability to support, supervise, and mentor nursing staff
Full-Time Registered Nurse (RN)
We are seeking a Full-Time Registered Nurse (RN) to provide compassionate hospice care to patients in their homes or care facilities. The RN will assess patient needs, manage symptoms, administer nursing care, and work with the hospice care team to develop and follow individualized care plans. This role also involves monitoring changes in patient conditions, educating patients and families, maintaining accurate medical records, and helping patients remain comfortable and supported throughout their hospice journey.
Qualifications
- Current, active Registered Nurse (RN) license in Texas
- Current Basic Life Support (BLS) certification
- Valid Driver’s License, automobile insurance, and a reliable vehicle; ability to travel to patient homes
- Previous nursing experience preferred
- Hospice, palliative care, or home health experience preferred
- Strong communication, organization, and clinical assessment skills
PRN Dietitian
We are seeking a PRN Registered Dietitian to provide nutritional support and guidance to hospice patients in their homes or care facilities as needed. The dietitian will assess patients’ nutritional needs, recommend appropriate dietary plans, and collaborate with the hospice care team to help manage nutrition-related concerns and promote comfort and quality of life. This role also involves educating patients and families about nutrition, documenting assessments and recommendations, and adjusting dietary guidance based on patients’ changing needs and goals of care among other duties.
Qualifications
- Current Registered Dietitian (RD/RDN) credential
- Relevant state licensure, if required
- Previous experience in clinical nutrition preferred
- Hospice, palliative care, or geriatric nutrition preferred, not required
- Strong communication and patient education skills
- Ability to collaborate with an interdisciplinary healthcare team
- Ability to travel to patient homes and care facilities as needed
Full-Time RN Case Manager
We are seeking a Full-Time RN Case Manager to provide compassionate, skilled nursing care to hospice patients in their homes or care facilities on weekdays with some on-call availability. This role involves assessing patient needs, developing and implementing individualized plans of care, administering medications and treatments, providing patient and family education, and communicating closely with physicians and the interdisciplinary care team. The RN Case Manager will document patient status and interventions accurately, supervise and delegate responsibilities as appropriate, participate in case conferences and team meetings, and take part in the on-call rotation as required.
Qualifications
- Valid RN License in the state of Texas (or compact state license)
- Certified in Basic Life Support (BLS)
- Minimum of an Associate Degree in Nursing (ADN) or higher
- Valid Driver’s License, Automobile Insurance, and a reliable vehicle
- Experience in hospice or palliative care preferred
- Strong clinical assessment and critical thinking skills
- Knowledge of medication administration, treatments, and symptom management
- Ability to participate in an on-call rotation as required
- Preferred Certification in Hospice and Palliative Care Nursing (CHPN)
Full-Time Licensed Vocational Nurse (LVN)
We are seeking a Full-Time Licensed Vocational Nurse (LVN) to provide compassionate nursing care to hospice patients in their homes or care facilities on weekdays with some on-call availability under the direct supervision of an RN. This role involves identifying and prioritizing patient needs, administering medications and treatments, anticipating changes or emergencies, documenting patient status and nursing interventions, and providing education to patients and families. The LVN will assist the RN in implementing individualized plans of care, communicate closely with the interdisciplinary team, participate in case conferences and team meetings, and take part in the on-call rotation as required.
Qualifications
- Valid LVN License in the state of Texas (or applicable compact privileges)
- Certified in Basic Life Support (BLS)
- Valid Driver’s License, Automobile Insurance, and a reliable vehicle
- Hospice, palliative care, or home health experience preferred
- Knowledge of medication administration and treatments within the LVN scope of practice
- Ability to participate in an on-call rotation as required
