Full-Time RN Case Manager

We are seeking a Full-Time RN Case Manager to provide compassionate, skilled nursing care to hospice patients in their homes or care facilities on weekdays with some on-call availability. This role involves assessing patient needs, developing and implementing individualized plans of care, administering medications and treatments, providing patient and family education, and communicating closely with physicians and the interdisciplinary care team. The RN Case Manager will document patient status and interventions accurately, supervise and delegate responsibilities as appropriate, participate in case conferences and team meetings, and take part in the on-call rotation as required.

Qualifications

  • Valid RN License in the state of Texas (or compact state license)
  • Certified in Basic Life Support (BLS)
  • Minimum of an Associate Degree in Nursing (ADN) or higher
  • Valid Driver’s License, Automobile Insurance, and a reliable vehicle
  • Experience in hospice or palliative care preferred
  • Strong clinical assessment and critical thinking skills
  • Knowledge of medication administration, treatments, and symptom management
  • Ability to participate in an on-call rotation as required
  • Preferred Certification in Hospice and Palliative Care Nursing (CHPN)

Full-Time Certified Nursing Assistant (CNA)

We are seeking a Full-Time CNA / Home Care Aide to provide compassionate personal care and support to hospice patients in their homes or care facilities on weekdays with some on-call availability. This role functions under direct instruction and supervision of an RN or therapist. Essential functions involve assisting patients with personal care, mobility, range-of-motion exercises, and other services essential to maintaining comfort, dignity, and a safe environment. The CNA/Home Care Aide will observe and report changes in patient condition to the Supervisor or RN, maintain accurate records of services provided, communicate with patients and families, and participate in patient care conferences to support continuity of care among other duties.

Qualifications

  • Current CNA certification in the state of Texas
  • Certified in Basic Life Support (BLS)
  • Valid Driver’s License, Automobile Insurance, and a reliable vehicle
  • A minimum of two (2) years in a CNA role in the home health setting is preferred
  • Hospice or palliative care experience preferred
  • Ability to assist with personal care, mobility, transfers, ambulation, range of motion, and exercise
  • Ability to meet the physical demands of direct patient care

Full-Time Licensed Vocational Nurse (LVN)

We are seeking a Full-Time Licensed Vocational Nurse (LVN) to provide compassionate nursing care to hospice patients in their homes or care facilities on weekdays with some on-call availability under the direct supervision of an RN. This role involves identifying and prioritizing patient needs, administering medications and treatments, anticipating changes or emergencies, documenting patient status and nursing interventions, and providing education to patients and families. The LVN will assist the RN in implementing individualized plans of care, communicate closely with the interdisciplinary team, participate in case conferences and team meetings, and take part in the on-call rotation as required.

Qualifications

  • Valid LVN License in the state of Texas (or applicable compact privileges)
  • Certified in Basic Life Support (BLS)
  • Valid Driver’s License, Automobile Insurance, and a reliable vehicle
  • Hospice, palliative care, or home health experience preferred
  • Knowledge of medication administration and treatments within the LVN scope of practice
  • Ability to participate in an on-call rotation as required
